Introducing the LA Peel - now available at The Laser Clinic

 

 

The LA peeling solution is a superficial to mid depth peel comprised of three different peeling agents in an ethanol base. Each agent acts in a slightly different manner, making the solution highly effective and predictable. This peeling agent provides significant visible exfoliation with minimal complications and is simple to use.

*Patients who are pregnant or lactating should not undergo an LA peel.*

An LA Peel smoothes and rejuvenates sun-damaged skin. It is indicated for patients with mild to severe acne, melasma, moderate wrinkling, and actinic damage. Multiple areas can be treated at the same time. Peeling the face, neck, hands, and upper chest is quite effective. This peeling agent is excellent for all skin types. Ethnic and darker skin types should exercise caution and offer the patient a series of light peels.

Adding successive layers to the skin controls the depth or level of the peel. During a peel, an even layer of the solution is applied and allowed to dry without neutralizing. The LA peel is self-limiting and is left on the skin. After the initial layer is dry, a second layer can be applied to achieve deeper penetration. White milky frosting occurs and signifies the endpoint for layers added. Even frosting signifies a uniform, even peel.

Pre-treatment

 

Pre-treatment

·        Patient should begin using the Theraderm Skin Renewal System twice daily, 2-4 weeks prior to the peel procedure. The system will begin repairing the skin surface even before the peel is done.

·        Patient should avoid sun exposure, and should use a sunscreen with an SPF 30. This is to ensure that the patient’s skin is not red or irritated before the peel, thus reducing the chances of hyperpigmentation.

·        It is not necessary to clean the skin surface with acetone or alcohol, but these can be used for further cleansing of deep wrinkle lines and crease lines, which allows for a slightly deeper peel in those areas.

·        Please note that if the skin is rubbed with acetone, some exfoliation occurs pre-peel, and the peel solution will cause erythema or redness associated with a deeper peel earlier in those areas.

Cautions & Contra indications

·        The LA Peel is contra indicated during pregnancy.

·        Patients with an allergy to Aspirin

·        Should not be used while a patient has an active herpes lesion

·        Retin A should be discontinued two weeks before an LA Peel

·        Ro Accutane should be discontinued at least 3 months prior to an LA Peel

What is in the LA Peel

 

The LA’s peel solution includes (essentially) 14% each of resorcinol, lactic acid, and salicylic acid. Each ingredient performs an important function in the peeling process.

Resorcinol

Resorcinol is a derivative of phenol that has been used as a peeling agent for over one hundred years. It was first introduced for topical application in 1882. In higher concentrations, resorcinol has been shown to cause a separation in the epidermis at the level of the stratum granulosum while inducing an inflammatory response. A relatively low concentration of resorcinol in the LA peel solution is less likely to cause complete exfoliation with a raw surface area. Resorcinol helps to provide a numbing effect and increases the depth of the lactic acid and salicylic acid penetration. Some down time may be associated with re-epithelialization, along with the occurrence of a “resorcinol membrane” or a greyish appearance of the skin approximately 24 hours post-peel. The grey-brown membrane lasts for 2-3 days until visible peeling occurs.

Lactic Acid

Lactic acid occurs naturally in human skin as a part of metabolic processes. Lactic acid in normal skin is partly responsible for maintaining the acid mantle of the skin at a pH of 5.5 to 6.0. Thus, there is no toxicity from the lactic acid itself. It is less irritating than other alpha hydroxy acids and is relatively gentle to the skin.

Several studies have shown that lactic acid has a natural moisturizing effect on the skin. In the 1960’s dermatologists noted that when lactic acid was used in a moisturizer, it enhanced absorption of the moisturizer into the skin. This characteristic enables the lactic acid to work in conjunction with the other two agents to enhance the evenness of the LA peel.

Salicylic Acid

Salicylic Acid penetrates directly into skin follicles, making it a very important component of the LA peel solution. As well as absorbing into the broad skin surface, this acid goes directly into the waxy buildup associated with acne. Salicylic acid unroofs comedones and pustules, opening the sebaceous gland and stimulating the elimination of wastes.

It is necessary to skin test a small area of the inner upper arm prior to applying a full-face peel. A skin test will help to eliminate patients who are sensitive to aspirin.

The peels have very little if any downtime and can be performed within a 30minute appointment. It usually takes 4-6 treatments at 2-3 week intervals to achieve the desired effect.

What skin types can be treated with Jan Marini?

The peels are suitable for all skin types including black and asian skin. It increases hydration and suppleness and is also used for treating irregular skin tones (mottling or pigmentation) and acne.
 
Glycolic Acid is derived from sugar cane and is therefore a natural derivitive.  There have been no cases of serious side effects reported but occasionally, there may be some redness - which is normal.
